Our multidisciplinary team provides coordinated inpatient rehabilitation for patients with complex HIV, homelessness, and detox-related needs, integrating medical, therapeutic, and psychosocial care.

Mildmay Hospital provides specialist inpatient rehabilitation for people with complex medical and psychosocial needs, including HIV-associated conditions, homelessness, and substance dependence.
 

Care is delivered through a structured multidisciplinary team (MDT) model, enabling coordinated assessment, treatment, and discharge planning within a single setting. This approach supports patients who are not best served by standard acute or community pathways.

Our patients often present with overlapping physical health, mental health, cognitive, and social needs. Care is therefore delivered through coordinated input from multiple disciplines, working to a shared care plan.

How We Work

Care is delivered through defined MDT processes:

  • Initial Assessment: Multidisciplinary assessment following admission

  • MDT Review Meetings: Weekly team meetings to review progress, risks, and care plans

  • Care Planning: Goal-focused plans developed with the patient, including clinical, functional, and social outcomes

  • Discharge Planning: Begins early and involves coordination with GPs, community teams, housing providers, and other services

Clinical Governance

Mildmay operates within established clinical governance frameworks:

  • Regulated by the Care Quality Commission (CQC)

  • Practice aligned with relevant NICE guidance and UK clinical standards

  • Ongoing audit, outcome monitoring, and quality improvement

Referrals & Enquiries

We accept referrals from healthcare professionals, primarily from London, but also from across the UK.

  • All referrals are initially reviewed by our Admissions Manager, who is medically trained and able to address most clinical and practical queries

  • Where required, cases are escalated to the Medical Director and MDT for further review

  • We aim to respond within 48 hours.

Role of Charitable Funding

While core clinical services are NHS-funded, charitable support enables elements of the MDT that are not routinely commissioned, including art therapy, enhanced psychosocial support, and pastoral care.

These services contribute to patient engagement, recovery, and longer-term stability.
